Checks issued less than one year ago: The payee or check holder shall apply to an agent institution of the National Treasury for loss report and stopping payment. B. Checks issued more than one year ago: The payee or check holder shall apply to the original issuing disbursement office for loss report and stopping payment. 2. National Treasury check reissuance: The payee or check holder is required to first complete the loss report and stopping payment procedure and then fill out the National Treasury Check Reissuance Application to apply with the original issuing disbursement office for reissuance.
